Modulo: 30736 - GERIATRIA

The course of Internal Medicine III consists of six modules (blocks): geriatrics (2), community medicine, hygiene, medical statistics and palliative care.
The geriatrics modules aims to provide:
Theoretical and practical knowledge of the pathophysiology, clinical and therapeutic approach of the main diseases of the elderly and geriatric syndromes.
Theoretical and practical knowledge of the clinical and diagnostic approach to older patients with particular reference to the multidimensional geriatric assessment
Description of age-related and gender-related differences in demography and age-related pathophysiologic changes to understand the relevance of aging patterns and determinants of the quality of life of an elderly person.
The module of community medicine aims to provide the knowledge of the general principles of the organization and structure of the national health system with specific focus on the organization of local health companies and the Department of primary care, addressing also the issue of territorial continuity of care. The role of the primary care physician and the pediatrician will be discussed.
The modules of Epidemiology and of Medical statistics aim at providing intermediate knowledge of epidemiology and biostatistics for the research and for the advanced interpretation of scientific articles, and to implement databases of observational studies and clinical trials.
The module of palliate care aims to provide basic knowledge on objectives and outcomes of palliative care; The course will describe and discuss the complex needs of the patients, and his/her family, with uncurable disease and will discuss ethical and legal implications of palliative care.

Knowledge and understanding
GERIATRICS AND COMMUNITY MEDICINE
At the end of the course the student will understand the main biological mechanism underlying the aging process and the most common geriatric syndromes. The student will also know the clinical and social characteristics of the geriatrics patient and the method of the comprehensive geriatric assessment, along with the concept of transition and continuity of care.
At the end of the course the student will know the organization of the organization of the National Health system, of the Local health companies, and of the department of primary care. The student will also understand the role of the primary care physician and of the pediatricians in the local health care system
HYGIENE AND MEDICAL STATISTICS
At the end of the course the student will:
- understand the role of epidemiological metho for health care research.
- understand statitical parameters to be used in clinical decision process;
- to interpret the results of multivariate analyses to identify risk factor for diseases in different population subgroups (i.e. sex and gender).
PALLIATIVE CARE
At the end of the course the student will:
-Understand the concept and basic principles of palliative care
-identify the complex needs of the patients with and without oncological diseases
-recognize social and comunication need of the patients and his/her family
- recognize pain, symptoms and most common condition of patients with untreatable disease
- understand etical and legal implication of palliative care

Applying knowledge and understanding:

-perform and correctly utilize the results of a basic comprehensive geriatric assessment;
-recognize the expressed and unexpressed needs of the geriatric patient and plan a tailored assistential plan
- properly utilize the different settings and service of the department of primary care to guarantee the optimal transition and continuity of care
HYGIENE AND MEDICAL STATISTICS
At the end of the course the student will have developed the ability to:
- read and interpret the advanced results of a scientific study;
- set up a data collection and the development of a database for a scientific study.

GENERAL AND APPLIED HYGIENE (EPIDEMIOLOGY):
Basic concepts on epidemiological methodology (recall), study designs, bibliographic research, main epidemiological measures.
General and advanced concepts on the interpretation of studies, with specific examples taken from the preventive medicine field.
Searching for the scientific evidence. Importance of the epidemiological approach for research and healthcare management.
Evidence-based medicine and distortion of the results due to conflicts of interest, and discussion of some concrete cases taken from the scientific literature

MEDICAL STATISTICS:
Diagnostic tests, screening programs, and related measures of efficacy and validity (advanced).
Concepts of statistical inference (advanced), variables, database creation methodology, synthesis and presentation of results, main statistical tests.
Problems of the statistical methodology used in scientific studies, critical reading and interpretation of the measures and of the tests used.
95% confidence intervals, statistical inference, and probability (advanced concepts)
Recall of the main epidemiological biases and their effects of analyses
Basic concepts of multivariate analysis

PALLIATIVE CARE
Palliative care and specific needs of patients with and without oncological diseases
Communications and patient-physician relationships in palliative care
Ethical problems in palliative care
Practical activity at Ferrara Hospice or Geriatric care unit
Presentation and discussion of complex clinical cases focused on the following issues:
pharmacological treatment of severe symptoms
use of standardized scales for outcomes evaluation
managment of refractory simptoms and palliative sedation
Difficult communication with the patients and his/her family
Psychological and spiritual needs of the patient
Informed consent and advanced directive

All topics will be analyzed according to gender difference to encourage a personalized clinical approach focused on differences and specific characteristics of the single patient

Learning assessment procedures

The learning verification will be carried out by a written exam, lasting 60 to 90 minutes, without the use of notes or books or personal multimedia support, in the presence of the teachers. The test will be formulated with 60 multiple-choice questions (4-5 multiple-choice) relating to all the educational goals. A penalty for wrong answers will be given. A minimum score of 18 out of 30 will be required to pass the exam. The maximum grade, equal to 30 cum laude, is obtained by those who will answer all the questions correctly. The number of questions on each topic will be proportional to the weight of the single module. The test will be administered either in a classroom or from remote (Google meet platform) according to the pandemic situation.
